The second part of the computerized (OMR) application form which must be filled by HB pencil (not pen) only.

The OMR application form will be processed by computer. Therefore the fo rm should be handled carefully to avoid damage to it and must not be folded.

The ovals must be fully darkened. Check them for completeness and accura cy. For filling in the category or the choice of centers, use the code given in the information brochur e. For date of birth, three segments, each of two cells, are provided for marking date, month and year of yo ur birth. If your birth date is : 7-3-1978, mark as 07 03 78 in the appropriate cells and darken the ova ls below the segments. Do not leave any column blank.
