Application Forms

Candidates should note that there are two sets of Application Forms to be filled in.

(1) Normal Application Forms which are to be processed manually. These Forms are to be filled up in ink only.

(2) Computerised (O.M.R.) Application Form which is to be processed by computer and should be handled very carefully. Do not fold, staple, pin, wrinkle, scribe, tear, wet or cause such damage to this sheet. For completing this form both ink and HB Pencil will be required.

Instructions for completing the two sets of Application Forms should be read carefully, before filling the Application Forms.
