• Three recent and identical photographs of size 3.5 cm x 4.5 cm are to be pasted at appropriate places in the forms as follows :

(i) On Application Form Page - This photograph must be attested on the front side by:

(a) The Head of the Institution where the candidate is currently studyimg as a regular Full Time student, OR

(b) Class I Gazetted Officer for the other candidates.

(ii) On Verification Form Page - photograph not to be attested.

(iv) On Admit Card Page - photograph not to be attested.

The candidates are advised to have some spare copies of the above photograph with them. The spaces for the candidate's Roll Number is not to be filled in by the candidate. The candidates are advised to note the application form number with them for any future reference.

Name: Write your name in full as entered in High School or equivalent ce rtificate in Capital Letters only.

Date of Birth: Write the date, month and year of your birth as per English calendar as recorded in High School or equivalent certificate, e.g. 24.05.1980.

Nationality : Write your nationality in the given space.

Sex : Tick [ ] whichever is applicable.

Father's Name : Write the name of your father legibly, in full, in capital letters.

Mother's Name : Write the name of your mother legibly, in full, in capital letters.

Aptitude Test Option : Tick [ ] YES or NO whichever is applicable . If YES, attach a crossed Demand Draft for Rs. 50.00 as perscribed under Application Form & Fee, Aptitude Test Fee .

Write Form number and your Name at the back of the draft.

Only those candidates who have paid Aptitude Test fee as specified are eligible to appear in this Test. Further only those candidates , who appear in the Aptitude Test, will be considered for admission to B.Arch. course.

Candidates should sign in the box provided just above the photograph.

Complete Mailing Address : Write the complete mailing address in capital letters including the PIN CODE in the boxes provided. Also write the telephone number if any with STD code.

Particulars of Examinations Passed / Appeared / Appearing be very clearly filled in giving all the details asked for . Those candidates who are pursuing any course of study after Intermediate Science / 10+2 / Equivalent examination must also give the details of the same.

Declaration : The declaration is to be signed by the candidatee as well as his / her father / guardian as the case may be. Father / Guardian should write name below signatures in Capital Letters. Unsigned forms will not be considered.

Verification Form / Admit Card : The entries on the Verification Form and the Admit Card page should be done very carefully. Candidate's normal signatures ( and not the name in capitals ) is to be put in the spaces provided for signatures. Ensure that you have written your complete Mailing Address in the space allocated in the Admit Card.

Acknowledgement Card : Canditate should fill in the acknowledgement card and affix Rs. 1.50 postal stamps.

Envelope marked Application Form for sendings Forms etc. : Candid ate should write form number, his / her name and code number and name of the first choice of the place of examination in the space provided on the envelope marked 'Application Form' and all the documents with application form must be sent in this envelope by registered post, speed post or courier service so as to reach REE-99 office by the last date. Forms sent by ordinary post will not be accepted.

One Envelope should contain only one Application Form alongwith the Relevant Documents.
